CME CREDIT

AOA

Grandview Medical Center is accredited by the American Osteopathic Association (AOA) to sponsor continuing medical education for physicians. The AOA designates this program as meeting the criteria for 25 credits in category 1-A.

CME

Attestation Certificate of Hours will be provided in your registration packet. Registrants must sign this form attesting to the total hours attended and return the form to the registration desk to be verified to receive credit. The verified copy of this form should be saved for your records. Only those physicians who return the form will be given CME credit for this program. Physicians who fail to return this form will not be given any CME credits as the FPRR certifies CME directly to the AOA.

AAFP

Application for CME credit has been filed with the American Academy of Family Physicians (AAFP). Approval for this program is pending.

State Medical Board of Ohio

All physicians may claim Category 1 credit for attending this conference.

Nursing

For purposes of recertification through ANCC, offerings will be accepted as part of the 50% requirement provided by organizations accredited through the Accreditation Council on Continuing Medical Education – Category I.

Take Exit 53 (3rd, Salem) and stay in LEFT lane.  Turn LEFT at the traffic signal (3rd St) and get in RIGHT lane.  At the third traffic signal (Perry St) turn RIGHT.  Next traffic signal is 4th St and turn RIGHT.  Go approx. 300 feet: Turn RIGHT into Lot C underground parking garage.

Take I-75 Northbound to Exit 53 (2nd, Salem, 1st) and stay in RIGHT hand lane.  Road curves slightly, at third traffic signal (Perry St) turn RIGHT.  Follow this to 4th St and turn RIGHT.  Go approx. 300 feet: Turn RIGHT into Lot C underground parking garage.

Take I-75 Southbound to Exit 51 (Edwin C. Moses Boulevard) stay in far LEFT lane, which circles back around to I-75 Northbound.  Take Exit 53 (2nd, Salem, 1st) and stay in RIGHT hand lane.  Road curves slightly and at the third traffic signal (Perry St) turn RIGHT.  Follow this to 4th St and turn RIGHT.  Go approx. 300 feet: Turn RIGHT into Lot C underground parking garage.

Refunds

A refund of one-half the total fees will be made by providing a written letter faxed to (844) 615-6005 by January 29, 2019. No refunds will be issued after January 29, 2019. If the Family Practice Review and Reunion Committee cancel a course for any reasons, all registration fees shall be returned in full.

Grievances

Programs shall state: Grandview Medical Center/Family Practice Review and Reunion events are designed to comply with Uniform Guidelines for Accrediting Agencies of Continuing Medical Education. Questions or complaints concerning the program should be directed to the Chairmen of the Family Practice Review and Reunion Committee. Additional information concerning compliance may be obtained by contacting the AOA Division of Continuing Medical Education at 1-800-621-1773. No credits will be given to physicians who fail to register.

The Continuing Medical Education Office at Grandview Medical Center expects that participants in CME activities should be made aware of any affiliation or financial interest that may affect the speakers’ presentation(s). All speakers have been requested to complete a conflict of interest statement.

Funding

Funds paid to and disbursed from the Annual Family Practice Review and Reunion for educational purposes, are done so as per agreements with commercial sponsors and also at the discretion of the Family Practice Review and Reunion Committee.
